Separate the visible stain from the odor problem
Mark which rooms are affected and whether odor changes when the carpet is damp. The company may need to inspect carpet backing, padding or nearby surfaces before suggesting a treatment. Do not accept a permanent-odor-removal promise without an explanation of what is being assessed.

Questions before you request
Does carpet cleaning always remove pet odor?
No. Odor may involve backing, padding or other materials. The company should explain what it can treat and whether inspection or another type of work is needed.
Can I request a different schedule?
Yes. Choose Emergency / ASAP, Today, This week or Flexible, and describe any access or completion deadline. No schedule is confirmed by this form.
Is a written quote guaranteed?
No. A company may not be found or may decline the job. If one considers the request, ask it to confirm the work and price before authorizing service.
